• The test will only take one night. Please perform the test within three (3) days of receiving your device.
  • You will take the home sleep test and return the Unit to HomeSleep by delivering the Unit using the box provided and putting the pre-paid label inside the box over the original label and return the box to a United Parcel Service ("UPS") shipping location or drop box no later than seven (7) days after receiving the Unit.
  • You understand that you are responsible for the Unit until the prepaid return address label is accepted and scanned into circulation by UPS.
  • You agree to return the equipment intact and undamaged and will be responsible for the repair/replacement cost of any damaged/destroyed equipment.
  • If you do not return the Unit to UPS within seven (7) days of receiving the Unit, you will be charged $25.00 per day until you deliver the Unit to UPS.
  • If you do not return the Unit within twenty-one (21) days of receiving the Unit or the Unit is lost or stolen while in your possession, you will be responsible for the full replacement value of $1,100.
  • HomeSleep provides health insurance benefit information that your insurance company provides to HomeSleep. This is not a guarantee of benefits and you understand that you are financially responsible for any health insurance deductibles, coinsurance or fees for noncovered services.
